TB Test Overview

All MTC volunteers must provide MTC and the MVLA High School District with proof of a negative TB Test by a licensed health care provider within the past four years or a waiver stating they should not need a test.

TB Test Options

If you don’t already have a recent test, you may obtain a TB test without cost by completing and printing this form and testing at the MVLA district’s provider, Concerta: MVLA TB Test Authorization Form; or

You can go to Cedar Crest Nursing and Rehabilitation Center which also offer a free test for MTC volunteers (see details below); or

Your physician may complete the CTCA Risk Assessment Form if they feel you should not need to take the test. Often, your physician can complete this form without you having to make an appointment.

Submit Test Results to MTC and MVLA

Please submit a copy of your results or waiver with your MVLA Volunteer Application form 

Please also send us a copy at: programs@mentortutorconnection.org.

Again, please make sure to do both: provide a copy to the MVLA District office – and – provide a second copy to Mentor Tutor Connection.

Cedar Crest Details

To have your test taken at Cedar Crest Nursing and Rehabilitation Center, please walk in any time Monday through Friday between 10 am and 5 pm. *You may want to call ahead at (408) 738-4880 to check their current wait times.

72 hours from your appointment, return to Cedar Crest to have your skin test read.  No appointment is necessary.  They will give you your TB Test Screening Results.
